Attached below is the Account Change Request Form.
Includes the newly added instructions for Seasonal Address Standing Instructions ("Snowbirds"; Complete Every Three Years).
Use the attached form for name changes, address, phone and e-mail changes for all client accounts. The form only requires a designated advisory support representative's signature to attest to the changes and to ensure the changes will be made across all custodians and platforms. Please note this form is "maybe" sufficient for processing the changes for all of Spire's preferred custodians. Please be sure to check with the direct business sponsor & custodians for their additional required forms to complete the account maintenance request. Keep in mind that other Companies & Direct Business Sponsors may require additional forms and their own procedures.
